Individualized Resident Care & Support

Our Wellness Director meets with new residents and their families to assess current healthcare needs and create a Personalized Service Plan. This plan may include resident or family concerns, as well as recommended assistance with daily activities or personal care services, all coordinated by our 24-hour on-site, professional caregivers.

We encourage open communication and teamwork between our residents, their families, physicians and staff to create the ultimate healthcare environment, now and over time.

Level of Care Defined

Level of Care Defined: Level of Care (LOC) generally refers to the amount of assistance an individual will need with the activities of daily living, as well as management of one’s health.

Examples of Activities of Daily Living

  • Dressing

  • Transfers
  • Continence care
  • Mobility
  • Grooming

How is Level of Care Determined

An assessment is provided by our community nurse using a standard point system to develop a specialized Service Plan for the resident. The goal is to develop a plan for the resident to meet all their specific needs – the points accumulated link to the Level of Care.

Level of Care Evaluations to Monitor Health Care Needs

  • Pre-assessment to determine what LOC resident requires.
    If resident moves in 30 days after the original evaluation,
    the wellness team will do a second evaluation to be sure all
    needs are being captured.
  • Move-In Assessment to determine resident LOC pre-
    assessment and/or if any changes are required for their best
    care
  • Routine Assessment provided every six months
  • Change of Condition Assessment provided upon return from
    extended hospital stay in order to confirm resident’s current
    LOC status (improved or declined in condition)

What are the 7 levels of care? The 7 levels represent a scale of care with 1 being the least comprehensive and 7 being the most. Level 1 for example means residents are mostly independent but may need reminders from care staff to perform activities of daily living and level 7 means residents have impairments that affect multiple activities of daily living requiring an extensive level of assistance from care staff as well as medication management.

Providing the most appropriate level of care for residents allows them to thrive, whether they are in assisted living, independent living, or memory care.
